HbA1c evaluation predicting hyperglycaemia and avoiding morbidity in steroid initiation

Maher Philip , Durkan Maeve

Introduction: Steroids can precipitate significant hyperglycemia and Diabetes Mellitus in vulnerable populations including DM, pre-DM and the elderly. Steroid risks include deterioration in DM, glucose toxicity, HONK and HHS. This may be avoided by a simple screening HbA1c, which could prompt a ‘safeguard algorithm’ for the patients including instruction in glucose monitoring. The ‘at risk’ cohort is identified, and a surveillance regime is implemented. </p...

The PCOS demographic in a dedicated University Hospital Clinic

Durkan Maeve , Kennedy Bernard

This retrospective review of all adult women referred to a dedicated University Clinic for presumed polycystic ovarian syndrome (PCOS) aims to determine i) the true PCOS prevalence and ii) to evaluate baseline demographics by diagnostic criteria and their relationship to metabolic and cardiovascular risk parameters. All charts were reviewed. The diagnosis of PCOS rests on excluding another endocrinopathy, and fulfilling any two of the three criterias as per the Rotterdam state...

Hyponatraemia in hospital care

Ó Murchú Oisín , Durkan Maeve

Introduction: Hyponatraemia (serum sodium <136 mmol/l) is associated with significant morbidity and mortality. International guidelines suggest a clear algorithm for investigation, inclusive of measurements of paired urine sodium and osmolality, thyroid function tests and a morning cortisol. The aim of this study was to prospectively investigate the assessment, management and clinical outcomes associated with hyponatraemia on hospital admission.Metho...

Devolving diabetes care to general practice

Hussain Arshad , Durkan Maeve , Alarrayed Hamed , Asfour Jude

In Ireland, we are devolving management of diabetes to primary care. Recent studies highlight the importance of early glycemic (metabolic memory) & multifactorial control. Our hospital‘new’patient waiting lists average 18 months and for ‘best practice’, this is “time lost”. We established a pilot program for a rapid assessment to reduce waiting times and ensure specialist input. This is a retrospective review of our 50 patients thus far. 30% w...
